- Active LPN license in the state of New Hampshire or compact state eligibility (NH LPN license required)
- Minimum 1 year of LPN experience; experience in long-term care, skilled nursing, or rehabilitation preferred
- Ability to work consecutive 12-hour day shifts and complete full contract duration
- Current CPR certification (BLS preferred)
- Strong communication, organization, and teamwork skills; experience with EMR documentation and medication administration Responsibilities
- Provide compassionate direct resident care in a long-term care / rehabilitation setting
- Administer medications and treatments safely and per physician orders
- Monitor, assess, and document residents' health status, changes, and progress in the EMR
- Collaborate with the interdisciplinary healthcare team—RNs, CNAs, therapists, and physicians—to implement care plans
- Support residents with activities of daily living (ADLs) and promote a positive, resident-focused environment
