The Licensed Practical Nurse (LPN) Care Coordinator provides comprehensive care coordination services to FellowshipLIFE members and clients across home and community-based settings. and serves as the primary point of contact for members and families, ensuring seamless coordination of healthcare services, community resources, and interdisciplinary communication to promote safe, high-quality, person-centered care.
ESSENTIAL FUNCTIONS
Coordinate services across the healthcare continuum to ensure timely, efficient, and member-centered care. Serve as the primary contact for assigned members and caregivers. Assist the RN Care Manager with implementation and monitoring of individualized care plans. Perform ongoing telephonic and in-person follow-up with members to monitor changes in condition and service needs. Coordinate physician appointments, specialist referrals, diagnostic testing, transportation, errands, and community resources. Facilitate smooth transitions following hospitalization, emergency department visits, skilled nursing stays, rehabilitation, or hospice services. Assist members and families in navigating healthcare services and available FellowshipLIFE programs. Identify barriers to care and communicate concerns promptly to the RN Care Manager.
ADDITIONAL RESPONSIBILITIES
Attend required meetings, case conferences, and continuing education. Travel to FellowshipLIFE communities and member homes as needed. Participate in rotating on-call coverage if needed. Maintain professional licensure and continuing education requirements. Demonstrate flexibility during periods of high census or organizational need. Perform other duties as assigned.
MINIMUM ACCEPTABLE QUALIFICATIONS
Current unrestricted Licensed Practical Nurse (LPN) license in New Jersey. Minimum two (2) years of clinical nursing experience. Experience in one or more of the following: Home Health, Care Management, Case Management, Geriatrics, Community-Based Services, Primary Care, Assisted Living Valid New Jersey driver's license with reliable transportation.
Preferred Experience:
Experience coordinating care for older adults. Experience with Medicare, Medicare Advantage, Managed Care, or chronic disease management. Knowledge of community resources available to older adults. Experience with electronic medical records and care management platforms.
Knowledge, Skills & Abilities:
Strong communication and relationship-building skills. Excellent organizational and time-management abilities. Ability to prioritize multiple tasks in a fast-paced environment. Strong customer service skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Critical thinking and problem-solving skills. Proficiency in Microsoft Office and electronic health records. Demonstrated commitment to person-centered care.
