Job Description
Job Summary We are seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide comprehensive home healthcare services. The ideal candidate will possess a strong clinical background, excellent patient assessment skills, and a commitment to delivering high-quality, personalized care in a home setting. This role offers an opportunity to make a meaningful difference in patients' lives by managing their health needs with professionalism, empathy, and precision. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and evaluation of medical conditions. Administer medications, injections, IV infusions, and other treatments following physician orders and safety protocols. Manage complex care plans for patients with chronic illnesses such as dementia, Alzheimer's disease, or developmental disabilities. Monitor patient progress and document medical records accurately using Electronic Health Records (EHR) systems like Epic or Cerner. Provide specialized care such as wound dressing changes, catheter care, tube feeding, and ventilator management when necessary. Educate patients and caregivers on medication administration, disease management, and safety procedures to promote independence. Coordinate with multidisciplinary teams for discharge planning, case management, and ensuring compliance with long-term care regulations. Maintain strict adherence to HIPAA guidelines and clinical confidentiality policies while delivering compassionate patient care. Skills Proven experience in critical care environments such as ICU or Level I Trauma Centers, with expertise in acute pain management and infectious disease care. Familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth for efficient documentation. Strong knowledge of medical terminology, ICD coding (ICD-9/10), and diagnostic evaluation techniques. Experience working with geriatric patients in settings such as skilled nursing facilities or assisted living communities. Skilled in performing procedures like venipuncture, catheterization, sterile processing, and first aid techniques. Ability to communicate effectively with patients of diverse backgrounds and coordinate with caregivers and healthcare teams. Knowledge of long-term care regulations, hospice care principles, and dementia or memory care strategies. Proficiency in physical examinations, medication administration, and patient assessment tailored to individual needs.
