In this role, you'll provide clinical care and ongoing health support to adults with intellectual and developmental disabilities living in residential homes throughout Ocean County.

Starting Pay:

Additional Shift Differentials:
Earn an additional $2.00-$4.00 per hour based on applicable shift assignments.
What You'll Do
Provide Clinical Nursing Care
Deliver nursing services within residential group homes while promoting the health and well-being of individuals receiving services.
Monitor changes in health status and respond appropriately to medical concerns and emerging healthcare needs.
Assist in developing, implementing, and evaluating individualized healthcare plans that support each person's unique medical needs.
Coordinate Comprehensive Care
Work closely with the agency Registered Nurse, residential leadership, physicians, specialists, pharmacies, and interdisciplinary team members to ensure coordinated healthcare services.
Accompany individuals to physician appointments, hospital visits, rehabilitation admissions, specialty consultations, and other medical appointments when needed.
Advocate for individuals by ensuring continuity of care and effective communication among healthcare providers.
Support Medication & Health Management
Maintain medication administration records, health assessments, and other clinical documentation with accuracy and attention to detail.
Support medication administration processes while ensuring compliance with agency policies, physician orders, and healthcare regulations.
Assist residential staff with health-related education, guidance, and nursing support as appropriate.
Promote Wellness & Quality Care
Encourage healthy lifestyles and proactive healthcare practices that enhance quality of life.
Participate in quality assurance initiatives, health education, and ongoing clinical training.
Help maintain safe, healthy residential environments where individuals can thrive while receiving person-centered care.

Qualifications
We're seeking licensed nursing professionals who combine clinical excellence with compassion and strong communication skills.
Must be at least 18 years of age.
Graduate of an accredited School of Nursing required.
Current Licensed Practical Nurse (LPN) license issued by the New Jersey Board of Nursing required and must remain in good standing.
Minimum of one (1) year of experience as a Licensed Practical Nurse required.
Valid New Jersey driver's license required with no more than five (5) motor vehicle points.
Strong communication, documentation, organizational, and clinical assessment skills.
Previous experience working with individuals with intellectual and developmental disabilities is preferred but not required.
