Licensed Practical Nurse (LPN) - Per Diem weekend Vitality Hospice & Palliative Care Join a team that believes compassion and comfort truly matter . Vitality Hospice & Palliative Care is seeking a dedicated Per Diem Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care to individuals receiving hospice services in their homes or care facilities. This per diem role offers flexibility, autonomy, and meaningful one-on-one patient care , making it ideal for nurses who want to make an impact while maintaining schedule freedom. Position Details Per Diem / As-Needed Flexible scheduling Field-based role with local travel Mileage reimbursement provided Work setting: patient residences and facilities Key Responsibilities Under the direction of an RN Case Manager and clinical leadership, the LPN will: Provide skilled hospice nursing care in accordance with the patient's individualized plan of care Perform nursing assessments and promptly report changes in patient condition Support pain and symptom management to promote comfort and quality of life Assist patients with activities of daily living (ADLs) as needed Educate patients, families, and caregivers on care needs and self-care strategies Accurately document visits and care provided in a timely manner Collaborate with the interdisciplinary hospice team to ensure coordinated, holistic care Participate in on-call rotation as available (optional, based on per diem needs) Qualifications Active and unencumbered LPN license in the state of practice Minimum 1 year of nursing experience preferred (hospice experience a plus) Knowledge of hospice philosophy and care for terminally ill patients Strong assessment, communication, and documentation skills Ability to work independently and manage time effectively Valid driver's license, reliable transportation, and current auto insurance Why Join Vitality Hospice & Palliative Care?
