Role:
We are seeking a dedicated and compassionate LPN Licensed Practical Nurse to join our interdisciplinary team at Spring Creek Healthcare Center. As an LPN, you will be responsible for providing high-quality nursing care to our patients and residents, under the supervision of our experienced nursing staff. You will work closely with our care team to develop and implement individualized care plans, monitor patient progress, and provide emotional support to our patients and their families.
Responsibilities:
Provide direct patient care, including administering medications, performing treatments, and conducting vital signs Assist with daily living activities, such as bathing, dressing, and ambulating patients Develop and implement individualized care plans, in collaboration with our interdisciplinary team Monitor patient progress, identify changes in condition, and report to nursing staff Provide emotional support and education to patients and their families Collaborate with other healthcare professionals to ensure comprehensive care Maintain accurate and detailed records of patient care and services
