Responsibilities:

Provide skilled nursing care according to the patient's individualized care plan Monitor patient condition and report changes to the clinical team Administer medications and complete routine health assessments Monitor vital signs and maintain accurate nursing documentation Provide assistance with mobility, personal care, hygiene, and daily routines Provide seizure monitoring and G-tube care Maintain a safe and supportive home care environment Communicate effectively with family members and healthcare professionals

Qualifications:
