Document all patient interactions thoroughly in electronic health records (EHR/EHR systems), ensuring compliance with HIPAA regulations and clinical confidentiality policies. Collaborate effectively with interdisciplinary teams to develop and implement personalized care plans. Requirements Valid licensure as a Licensed Practical Nurse (LPN) in New Jersey. Proven experience working with geriatric patients in Assisted Living setting. Demonstrated proficiency with EMR/EHR systems. Excellent communication skills for effective caregiver communication.
