Responsibilities Provide direct skilled nursing care in the patient's home in accordance with the established plan of care and within the LPN scope of practice. Perform skilled nursing treatments and procedures as ordered, including medication administration, wound care, respiratory treatments, and other prescribed therapies . Provide tracheostomy care , including routine trach care, suctioning, monitoring respiratory status, maintaining airway safety, and recognizing signs of respiratory distress or complications. Provide care for patients receiving enteral nutrition through gastrostomy (G-tube) and jejunostomy (J-tube) feeding tubes. Administer continuous and bolus tube feedings as ordered and according to the patient's individualized plan of care. Perform appropriate G-tube and J-tube site care , monitor tube placement and patency as applicable, administer prescribed medications through feeding tubes, provide ordered flushes, and monitor for complications or feeding intolerance. Operate and monitor enteral feeding pumps and related equipment according to the plan of care and manufacturer's instructions. Monitor and document vital signs, respiratory status, symptoms, intake/output, and patient responses to care and treatments . Assist with activities of daily living (ADLs) , including bathing, dressing, grooming, toileting, positioning, transfers, and mobility support as indicated by the plan of care. Communicate effectively with the Registered Nurse (RN), physicians, supervisors, caregivers, and interdisciplinary care team regarding patient status and significant changes in condition. Complete accurate and timely clinical documentation in accordance with the patient's plan of care, agency policies, payer requirements, and applicable New Jersey regulations. Educate and reinforce teaching with patients and families regarding medications, treatments, equipment, disease processes, infection prevention, and the established plan of care , within the LPN scope of practice. Maintain a safe and clean patient environment while following infection prevention, standard precautions, emergency procedures, and patient safety protocols . Recognize and respond appropriately to emergencies and changes in patient condition , including respiratory distress, airway obstruction, feeding intolerance, aspiration concerns, seizures, and other acute changes. Follow established emergency protocols, including notifying the supervising RN and/or emergency medical services when appropriate. Support continuity of care by participating in required case updates, supervisory communication, care coordination, and shift-to-shift reporting . Maintain patient confidentiality and comply with HIPAA, agency policies, and applicable federal and New Jersey healthcare regulations .
Pay:
$32.00 - $45.00 per hour
