Responsibilities Deliver comprehensive patient care by performing physical examinations, vital signs assessments, and patient evaluations with precision and empathy. Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Assist with medical procedures such as catheterization, suturing, tube feeding, and sterile processing to support clinical needs. Maintain accurate medical documentation using Electronic Health Record (EHR) systems like Epic, Cerner, or Athenahealth while ensuring compliance with HIPAA and confidentiality policies. Conduct patient assessments for diverse conditions including dementia, Alzheimer's disease, infectious diseases, acute pain management, and trauma cases. Collaborate effectively with multidisciplinary teams on discharge planning, case management, and care coordination for patients in skilled nursing facilities or home care settings. Support emergency response efforts by participating in triage activities, airway management, first aid procedures, and CPR when necessary. Experience Proven experience working as a Licensed Practical Nurse in various healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, outpatient clinics, or home health agencies. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for accurate medical records management. Critical care experience including ICU or emergency room settings is highly desirable. Familiarity with specialized care areas such as geriatrics, pediatrics (including neonatal ventilator management), hospice care, and behavioral health is preferred. Knowledge of

coding systems for diagnostic evaluation and medical documentation accuracy. Experience working with individuals with disabilities or developmental delays enhances your ability to deliver personalized patient care. Certification in Basic Life Support (BLS), First Aid, Phlebotomy skills, and IV insertion are required to ensure readiness for diverse clinical situations.
