Job Summary:

We are seeking a dedicated and compassionate LPN/RN to join our healthcare team. The successful candidate will provide high-quality patient care in a dynamic and fast-paced environment. The ideal candidate will have experience in home healthcare and working with the pediatric population.

Scheduling needs:

MIDDAY :

4PM-9PM

MON-FRI AND 12HRS SHIFT SAT AND SUN MONDAY THROUGH FRIDAY

Responsibilities:

• Provide direct patient care, including administering medications, performing wound care, and assisting with treatments.
• Conduct physical examinations and document patient vital signs, medical histories, and observations.
• Utilize electronic health records (EHRs) to update patient information and track treatment progress.
• Collaborate with healthcare providers to develop and implement patient care plans.
• Provide education and support to patients and their families regarding their condition, treatment options, and prevention strategies.
• Maintain a safe and clean environment, adhering to infection control protocols and other safety standards.
• Monitor and report patient progress, including any changes or concerns.

Experience:

• A current LPN license in the state of employment.
• Experience working in home healthcare settings.
• Familiarity with EHR systems.
• Strong communication and interpersonal skills.
• Ability to maintain confidentiality and adhere to HIPAA guidelines.
• Experience with gtube/trach/vent highly preferred
