BAYADA Home Health Care is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hudson County Visits team. In this role, you'll provide one-on-one skilled nursing visits to adult clients throughout Hudson County, NJ , helping them safely remain in the place they call home. As a valued member of our interdisciplinary care team, you'll deliver high-quality clinical care while building meaningful relationships with your clients. A minimum of one year of clinical experience as a Licensed Practical Nurse is required. Responsibilities Provide skilled nursing visits to clients throughout designated territories. Administer medications, wound care, treatments, and other physician-ordered nursing interventions. Monitor client conditions and communicate changes to the Clinical Manager and Client Services Manager. Complete comprehensive nursing assessments as needed. Execute and document physician orders accurately and in a timely manner. Coordinate care with physicians and the interdisciplinary care team. Maintain accurate documentation of client assessments, interventions, and outcomes using BAYADA's electronic medical record (EMR) system. Qualifications Current New Jersey Licensed Practical Nurse (LPN) license. Minimum of one year of recent, verifiable clinical experience as an LPN. Graduate of an accredited nursing program. Home health experience strongly preferred, but not required. Ability to work independently and manage time effectively. Strong communication, organizational, and interpersonal skills. Experience with electronic medical records (EMR) preferred. Reliable transportation and ability to travel throughout the assigned territory. Why BAYADA?
