Job Description Job Description Description Axia Care Center is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our team in providing high-quality, person-centered care to our long-term care residents. As an LPN, you will be responsible for delivering nursing care within the scope of practice, and in accordance with facility policies, procedures, and state regulations. Current CPR/BLS certification Key Responsibilities Administer medications and treatments according to physician orders and facility protocols Monitor and document residents' health status and vital signs Provide wound care and perform routine nursing procedures Assist with admissions, discharges, and care plan implementation Maintain complete and accurate nursing records and documentation Respond promptly to changes in resident condition and communicate as needed Ensure compliance with all safety, infection control, and health regulations Collaborate with CNAs and other facility staff to support residents' daily needs Participate in in-services, training, and quality assurance activities Benefits Competitive pay Flexible scheduling Supportive team environment Training and development opportunities Benefits available based on employment status
