Job Description

Overview Join our dedicated healthcare team as a Licensed Practical Nurse (LPN) specializing in Home Care, where your compassionate care and clinical expertise will make a meaningful difference in patients' lives. In this vital role, you will provide personalized nursing services directly in patients' homes, promoting health, safety, and independence. Your energetic approach and commitment to excellence will help foster a supportive environment for individuals of all ages, including seniors and those with complex medical needs. This position offers an exciting opportunity to apply your diverse clinical skills in a dynamic, patient-centered setting. Duties Deliver comprehensive nursing care to patients in their homes, including medication administration, wound care, and vital sign monitoring Perform detailed patient assessments to evaluate health status and develop tailored care plans Manage IV infusions and catheter care with precision and adherence to safety protocols Coordinate with interdisciplinary teams using EMR (Electronic Medical Records) systems such as Epic or Cerner to document care accurately and efficiently Educate patients and caregivers on health management, medication use, and disease prevention strategies Conduct triage assessments over the phone or in person to determine urgent needs and prioritize care accordingly Assist with diagnostic evaluations, specimen collection, and processing as needed for ongoing patient monitoring Experience Valid Licensed Practical Nurse (LPN) license with current state certification Prior experience working in home care or community health environments preferred Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth Critical care experience including ICU, emergency medicine, or Level I Trauma Center is highly desirable Experience working with individuals with developmental disabilities, dementia (including Alzheimer's and memory care), or behavioral health conditions is a plus Knowledge of long-term care regulations and skilled nursing facility standards is advantageous Familiarity with medical procedures such as IV insertion, catheterization, sterile technique, and basic life support (BLS) certification Join us to provide exceptional patient-centered care that empowers individuals to live healthier lives at home. Pay:
$28.02 - $33.75 per hour Expected hours: 20.0 - 40.0 per week
