Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Athenahealth. Administer medications, injections, IV infusions, and other treatments following strict protocols and safety guidelines. Assist with complex procedures including catheterization, tube feeding, wound suturing, and sterile processing techniques. Monitor patients' conditions closely, manage acute pain, and provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments. Support triage activities by evaluating patient needs in urgent care or emergency room settings; coordinate referrals and discharge planning efficiently. Maintain accurate medical documentation in compliance with HIPAA regulations and long-term care standards. Collaborate with multidisciplinary teams to ensure optimal patient outcomes in skilled nursing facilities, hospitals, assisted living communities, or home care environments. Experience Valid LPN license with current state certification required; prior experience working with geriatric patients preferred. Demonstrated proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth; experience working with medical imaging or medical records management is a plus. Critical care experience including ICU or Level I/II trauma center exposure highly desirable; familiarity with acute care and hospital medicine essential. Knowledge of ICD-9 and ICD-10 coding systems for accurate documentation; experience in hospice care or infectious disease management is advantageous. Hands-on experience with IV insertion, medication administration, venipuncture, and aseptic techniques required; familiarity with ventilator management and airway procedures is beneficial. Strong understanding of physiology, anatomy, and medical terminology; ability to perform physical examinations and diagnostic evaluations confidently. Excellent caregiver communication skills with experience working with individuals with disabilities or behavioral health needs; ability to handle emergency situations calmly and effectively.
