Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems. Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Assist with complex medical procedures such as catheterizations, tube feedings, wound suturing, and sterile processing techniques. Monitor patients' conditions closely, manage acute pain, and respond promptly to emergencies such as airway management or ventilator issues. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with diverse needs including geriatrics, dementia care, Alzheimer's care, pediatric cases, and individuals with developmental disabilities. Maintain strict adherence to HIPAA regulations and clinical confidentiality policies while documenting all patient interactions accurately within EMR/EHR systems. Educate patients and their families on health coaching topics like medication management, nutrition, discharge planning, and post-acute care instructions. Experience Proven experience working as an LPN in various healthcare environments such as hospitals (including Level I Trauma Centers), skilled nursing facilities, assisted living communities, or home & community care settings. Demonstrated proficiency in critical care areas including ICU, emergency medicine, urgent care, or trauma centers with familiarity in

ICD-9/ICD-10

coding for accurate medical documentation. Hands-on experience with specialized procedures such as phlebotomy, venipuncture, catheterization, sterile technique, and specimen collection/processing. Knowledge of advanced patient assessment techniques for complex cases involving infectious disease care, pain management, hospice care, or cardiopulmonary interventions like cardiac catheterization or spinal taps. Familiarity with EMR/EHR platforms such as Cerner or Athenahealth to ensure precise recordkeeping and compliance with long-term care regulations. Ability to work effectively with diverse populations including geriatric patients requiring memory or dementia care; pediatric patients; individuals with disabilities; or those needing acute or post-acute interventions. Strong understanding of anatomy and physiology principles combined with excellent caregiver communication skills to deliver compassionate patient-centered care across multiple settings.
