Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform personalized care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Assist with diagnostic evaluations such as specimen collection, laboratory testing, and medical imaging procedures. Support patients with activities of daily living (ADLs), including tube feeding, wound dressing, and mobility assistance using equipment like Hoyer lifts. Manage electronic health records (EHR/EHR systems) to document patient information securely and maintain compliance with HIPAA and long-term care regulations. Provide specialized care for diverse populations such as geriatric patients, individuals with developmental disabilities, dementia or Alzheimer's disease, and those requiring hospice or palliative care. Collaborate with multidisciplinary teams on discharge planning, case management, and emergency triage to ensure seamless patient transitions and optimal outcomes. Requirements Current licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction. Proven experience working in medical-surgical units, long-term care facilities, hospitals (including ICU or trauma centers), or home health settings. Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for accurate documentation. Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), CPT coding, and infection control protocols. Hands-on experience with IV insertion, medication administration (including injections), phlebotomy, and sterile techniques. Ability to perform physical examinations and assessments with attention to detail in high-pressure environments like emergency rooms or urgent care clinics. Strong communication skills for effective caregiver interaction and patient education across diverse populations including pediatrics and geriatrics. $45.00 - $50.00 per hour Expected hours: 36.0 per week
