Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our long-term care team. The ideal candidate will possess a strong commitment to providing high-quality patient care in a supportive and respectful environment. As an LPN in this role, you will be responsible for delivering essential nursing services to residents, ensuring their comfort, safety, and well-being. LPN LONG TERM CARE
Shift:

Hours Nights Shift Details:
6.00 PM to 6.30 AM 36 hours/week. (3
  • 12) 9-week contract Perform comprehensive patient assessments, monitor vital signs, and document clinical observations accurately within electronic health record (EHR) systems.
Administer medications, including injections and IV infusions, following established protocols and safety procedures. Assist with activities of daily living (ADLs), such as bathing, dressing, and mobility support, ensuring resident dignity and comfort. Manage wound care, including dressing changes and infection control measures, particularly in cases involving complex or chronic wounds. Collaborate with physicians and interdisciplinary team members to develop and implement individualized care plans tailored to residents' needs. Provide specialized care for residents with dementia or Alzheimer's disease, including memory care strategies and behavioral management. Ensure compliance with long-term care regulations, HIPAA confidentiality policies, and safety standards at all times. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current registration in good standing. Prior experience working in long-term care facilities such as nursing homes or skilled nursing facilities is highly preferred. Demonstrated proficiency with EMR/EHR systems like Epic or Cerner for accurate documentation. Experience in managing complex patient needs including IV infusion, catheterization, pain management, and respiratory support such as ventilator care. Knowledge of geriatrics, dementia care, hospice services, and infectious disease protocols is essential. Strong communication skills with the ability to effectively collaborate with residents, families, and healthcare team members. Certification in Basic Life Support (BLS) or CPR is required; additional certifications such as IV certification or first aid are advantageous.
