LPN LONG TERM CARE

Shift:

- 10 Hours (day/Eve, Eve/Night or Flex to all Schedule will be Sun-Wed or Thurs-Sun 40 hours/week. (4

• 10) 13-week contract Conduct comprehensive patient assessments, monitor vital signs, and document clinical observations accurately within electronic health record (EHR) systems.

Administer medications, including injections and IV infusions, following established protocols and ensuring proper documentation. Assist with activities of daily living (ADLs), such as bathing, dressing, and mobility support, tailored to individual resident needs. Collaborate with interdisciplinary teams to develop and implement individualized care plans that address medical, psychological, and social needs. Manage acute pain and dementia-related behaviors through appropriate interventions, including Alzheimer's care and memory support strategies. Ensure infection control practices are maintained rigorously to prevent the spread of infectious diseases within the facility. Provide education to residents and their families regarding health conditions, medication management, and post-discharge instructions. Maintain compliance with long-term care regulations, HIPAA confidentiality policies, and safety standards at all times. Experience Valid licensure as a Licensed Practical Nurse (LPN) with current state certification. Prior experience working in long-term care facilities or skilled nursing facilities is highly preferred. Demonstrated proficiency with EMR/EHR systems such as Cerner or eClinicalWorks for accurate medical documentation. Experience administering IV infusions and managing catheter care is desirable. Background in geriatrics or working with elderly populations is essential; familiarity with dementia care and Alzheimer's support is advantageous. Knowledge of ICD coding (ICD-9/ICD-10) for accurate documentation of diagnoses. Proven ability to work effectively within multidisciplinary teams in fast-paced healthcare environments such as hospitals or critical care units.
