LN01 Job Summary An LPN in a rehabilitation setting provides nursing care to patients recovering from illness, injury, surgery, or chronic conditions. The LPN works under the direction of an RN or healthcare provider and collaborates with physical, occupational, and speech therapists to support patients' recovery and independence.

Responsibilities Duties:

Provide direct nursing care to rehabilitation patients according to the plan of care. Monitor and document patients' vital signs, symptoms, pain levels, and overall condition. Administer medications and treatments as ordered by physicians and according to facility policies. Assist patients with activities of daily living (ADLs), including bathing, dressing, toileting, and mobility. Support patients during physical, occupational, and speech therapy activities as appropriate. Perform wound care, dressing changes, catheter care, and other nursing procedures within LPN scope of practice. Observe patients for changes in condition and promptly report concerns to the RN or healthcare provider. Educate patients and families about medications, treatment plans, safety, and rehabilitation goals. Maintain accurate and timely nursing documentation and patient records. Follow infection-control, fall-prevention, safety, and rehabilitation protocols. Communicate effectively with nurses, physicians, therapists, patients, and families. Promote patient independence, dignity, comfort, and safe mobility. Participate in care planning and interdisciplinary team meetings as required.
