Position Summary:

We are seeking a compassionate, dependable, and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate is committed to providing high-quality, patient-centered care and works well as part of a collaborative healthcare team.

Key Responsibilities:

Provide compassionate and professional nursing care to patients/residents. Administer medications and treatments as ordered by physicians and according to facility policies. Monitor and document patients' health status, vital signs, and changes in condition. Assist with activities of daily living and provide appropriate patient support. Perform nursing assessments and communicate changes in condition promptly. Maintain accurate and timely patient records and documentation. Collaborate with physicians, RNs, CNAs, therapists, and other healthcare professionals.

Requirements:

. Active, unrestricted certification/licensure as

LVN / LPN.

Active BLS from

AHA. COVID

card or Medical / Religious Exemption Demonstrated ability to provide top-tier service and exceptional communication skills.
