Job Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, skilled nursing services in our 62-bed SNF. Your energetic approach and dedication will help ensure patients receive high-quality, personalized care while maintaining compliance with healthcare regulations. Shifts 3pm-11pm 11pm-7am Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Point Click Care. Administer medications and injections accurately following physician orders, including IV infusions and catheter care. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures. Manage acute pain and chronic conditions through appropriate interventions, including wound care, tube feeding, and ventilator management. Provide specialized care for patients with dementia or Alzheimer's disease in memory or geriatric care settings while ensuring safety and dignity. Support discharge planning and coordinate with case management teams to facilitate smooth transitions of care across settings. Maintain strict adherence to HIPAA regulations and clinical confidentiality policies while documenting medical records accurately in PCC. Skills Proven experience working with diverse elderly patient populations. Strong knowledge of medical terminology, anatomy, physiology. Proficiency in using PCC. Knowledge of infection control practices and experience managing infectious disease cases; familiarity with ICD-10 coding enhances documentation accuracy. Excellent caregiver communication skills to educate patients and families about treatment plans and health coaching strategies. Qualifications Proven experience within healthcare and rehab units. Excellent organizational skills with the ability to oversee multiple patients Valid nursing license and relevant certifications demonstrating professional competency
