Role:

We are seeking a dedicated and compassionate LPN to join our team on a per diem basis, working every Friday and Saturday from 11pm-7:15am. As an LPN at Gurwin Jewish ~ Fay J. Lindner Residences, you will have the opportunity to provide exceptional care to our residents while enjoying a unique and luxurious assisted living community.

Responsibilities:

Provide direct patient care to our residents, including administering medications, performing vital signs, and providing wound care Develop and implement individualized care plans in collaboration with the healthcare team Maintain accurate and detailed patient records Participate in interdisciplinary team meetings to ensure comprehensive care Collaborate with other healthcare professionals to ensure seamless transitions and quality care Provide education and support to residents and their families Maintain a safe and clean environment for residents and staff

Requirements:

Current LPN licensure in the state of New York Prior experience in long-term care or assisted living preferred Strong clinical skills and knowledge of geriatric care Excellent communication and interpersonal skills Ability to work effectively as part of a team How to
