Job Description
Job Summary We are seeking dedicated and compassionate Licensed Practical Nurses (LPN) to join our dynamic healthcare team as a staff nurse in our Ambulatory Care and/or Specialty clinics and/or Nursing Home & Rehabilitation centers within the Bassett Healthcare Network. These are vital roles to our patients and you will have the opportunity to provide high-quality patient care across a variety of settings, ensuring comfort, safety, and well-being for diverse patient populations. Your expertise in nursing practices, combined with your enthusiasm for patient-centered care will contribute to positive health outcomes and an exceptional care experience. Responsibilities Provides direct and indirect nursing care individualized to the patient's needs Evaluates health data to identify patient problems Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures Advocates for patients to ensure that care preserves and protects the patient's autonomy, dignity and rights Document all patient interactions thoroughly using electronic health record (ie: EPIC) ensuring compliance with medical documentation standards. Collaborate with multidisciplinary teams Support discharge planning and case management efforts to ensure seamless transitions across outpatient specialty, and nursing home and rehabilitation services. Skilled in patient assessment, vital signs monitoring, pain management Excellent communication skills for triage situations, health coaching, family education, and interdisciplinary collaboration.
