Licensed Practical Nurse (LPN) Join our evening shift team at Crown Park Rehabilitation and Nursing Center! We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift team at Crown Park Rehabilitation and Nursing Center, a 200-bed Rehabilitation and Nursing Facility located in Cortland, New York. As an LPN, you will play a vital role in delivering high-quality patient care and ensuring the well-being of our residents. Join our team and experience the difference for yourself! About the
Role:
As an LPN at Crown Park Rehabilitation and Nursing Center, you will be responsible for providing direct patient care, administering medications, and performing various treatments under the supervision of our experienced medical team. You will also have the opportunity to build relationships with our residents and their families, as you work together to develop personalized care plans that meet their unique needs. We pride ourselves on creating a homelike environment, and we're looking for someone who shares our passion for delivering exceptional customer service.
Responsibilities:
Provide direct patient care to residents on the evening shift, including bathing, dressing, and grooming Administer medications and treatments as prescribed by our medical team Perform various medical procedures, such as vital sign checks and wound care Develop and implement individualized care plans with our interdisciplinary team Maintain accurate and detailed records of patient care and medications Collaborate with our rehabilitation team to ensure smooth transitions for patients returning home Participate in performance improvement initiatives to ensure high-quality care And many other responsibilities that will challenge and reward you!
