Overview Join our dedicated healthcare team as a Private Duty Home Care Nurse (LPN), where you will play a vital role in delivering personalized, compassionate nursing care directly in patients' homes. As an integral part of our home care services, you will provide skilled nursing interventions, promote independence, and support families through comprehensive care plans tailored to each patient's unique circumstances. Responsibilities Perform detailed patient assessments, including vital signs, physical examinations, and comprehensive health evaluations to determine individual care needs. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Manage complex medical procedures such as tube feeding, wound suturing, and ventilator management while adhering to sterile techniques and infection control policies. Monitor patients' conditions closely, recognizing signs of acute pain or deterioration, and implement appropriate triage protocols to escalate care when necessary. Document all patient interactions thoroughly, ensuring compliance with HIPAA and long-term care regulations. Educate patients and their families on medication administration, disease management—including Alzheimer's and dementia care—and safety procedures to foster independence at home. Coordinate discharge planning, case management activities, and collaborate with physicians or multidisciplinary teams to optimize patient outcomes across various settings such as skilled nursing facilities or assisted living communities. Qualifications Valid Licensed Practical Nurse (LPN) license with current state certification. Demonstrated experience working with individuals with developmental disabilities, memory care needs, or in hospice settings. Strong knowledge of physiology, anatomy, medical terminology, ICD coding (ICD-9/10), and medication administration protocols. Ability to perform complex procedures including trach care, ventilator support, colostomy care, wound care, catheterization, phlebotomy, and basic life support (BLS). Excellent caregiver communication skills coupled with a compassionate approach to senior care and family education.
