St. Xavier Community Services - Forest Hills, NY Immediate Hire St. Xavier Community Services is seeking a compassionate, dependable Licensed Practical Nurse (LPN) to join our newly established Opioid Treatment Program (OTP) in Forest Hills, Queens. The LPN will work as part of our multidisciplinary clinical team and primarily support our medication dosing operations for individuals receiving treatment for opioid use disorder.

Monday-Friday:

7:00 AM-1:00

PM Saturday:

7:00 AM-10:00 AM The position requires reliable early-morning availability and Saturday coverage as scheduled. Responsibilities Assist with the safe and efficient operation of the OTP medication dosing area. Administer/dispense medications within the LPN's authorized scope of practice and in accordance with applicable orders, policies, and regulatory requirements. Verify patient identity and medication orders prior to medication administration. Observe patients and promptly report clinical concerns, adverse reactions, or changes in condition to the RN, Nurse Practitioner, or Medical Director. Complete accurate and timely medication and nursing documentation. Maintain medication accountability and assist with required medication inventory, counts, and records. Follow controlled-substance security and medication-handling procedures. Support take-home medication processes as applicable. Obtain vital signs and assist with other nursing functions within scope of practice. Maintain patient confidentiality and comply with HIPAA and applicable substance use disorder confidentiality requirements. Work collaboratively with RNs, the Nurse Practitioner, Medical Director, counselors, and other members of the OTP team. Maintain a professional, respectful, and nonjudgmental approach when working with individuals receiving substance use disorder treatment. Follow all St. Xavier Community Services policies and applicable OASAS, SAMHSA, DEA, and federal/state requirements . Qualifications Current and unrestricted New York State LPN license required. Current CPR/BLS certification preferred. Experience in an OTP, substance use disorder treatment program, behavioral health, addiction medicine, or medication administration setting strongly preferred. Experience working with methadone and/or buprenorphine treatment is a plus. Strong attention to detail and medication-safety practices. Ability to maintain accurate documentation and confidentiality. Dependable and able to work early morning dosing hours. Strong communication skills and ability to work effectively as part of a multidisciplinary team.
