Job Description

JOB PURPOSE
The purpose of this job is to identify, track and provide care management/referral management services for the high-risk patients with an identified number of Chronic Diseases in the Primary/Specialty Care population. Employee also works directly with community and behavioral health agencies outside the Nathan Littauer Hospital Primary/Specialty Care system to coordinate services for this identified high-risk population, as determined by current applicable Quality Programmatic Metrics.
EDUCAITON
Possess a high school diploma or equivalent education. Graduation from an accredited Licensed Practical Nurse program or completion of required components to satisfy LPN licensing examination.
LICENSURE/CERTIFICAITON/REGISTRAITON
Possess a current New York State license for a Licensed Practical Nurse.
EXPERIENCE
Two (2) years of practice as an
L.P.N. PREFERRRED
Possess knowledge of regulations of chronic care management for primary care patients.
