The Licensed Practical Nurse will perform the following tasks: Provide direct nursing care to clients under the supervision and direction of clinical leadership. Maintain patient confidentiality in accordance with Phoenix House policies and procedures. Oversee unlicensed assistive personnel (e.g., CNAs, Medical Assistants, and Medical Technicians). Perform routine nursing procedures such as EKGs, wound care, and specimen collection. Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub-Q, intradermal) in accordance with NYS scope of practice and facility policies. Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership. Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership. EDUCATION

REQUIRED EXPERIENCE

At least three years of related experience Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and or drugs
