Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for the Evening and Nights Shift to join our healthcare team. Will consider double shifts for right candidates. The ideal candidate will possess a strong foundation in patient care, with experience across various clinical settings, including acute care, long-term care, and assisted living environments. As an LPN, you will play a vital role in delivering high-quality nursing services, supporting patient recovery, and ensuring safety and comfort for diverse populations. Your expertise in medical documentation, medication administration, and patient assessment will contribute significantly to our mission of providing exceptional healthcare. Evening shift is 3 pm to 11:30 pm and Night shift is 11 pm to 7:30 am. Duties Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating physiological responses to treatment. Administer medications and injections accurately, including IV infusions and catheter care, following established protocols and HIPAA regulations. Document all patient interactions and clinical procedures meticulously within electronic health record (EHR) systems such as Sigma Care or PCC Assist with medical procedures including tube feeding, venipuncture, and sterile processing techniques while maintaining aseptic technique standards. Support patients with complex needs such as dementia care, Alzheimer's care, memory care, and individuals with developmental disabilities or behavioral health conditions. Collaborate with multidisciplinary teams to develop discharge planning strategies and coordinate post-acute or home & community care services. Provide emergency response support through airway management, acute pain management, and basic life support (BLS) during urgent situations in compliance with hospital protocols. Engage in patient education on health coaching topics such as nutrition, medication management, and chronic disease management to promote wellness. Qualifications Valid LPN license with current state licensure; additional certifications such as BLS or ACLS preferred. Demonstrated experience in diverse clinical environments including long term care, hospital medicine, home care. Proficiency with EMR/EHR systems like Sigma Care Prior experience working with individuals with disabilities or in specialized settings such as hospice care, home care or geriatrics is highly desirable. Strong knowledge of anatomy, physiology, pharmacology, and medical terminology necessary for accurate patient monitoring and documentation. Ability to work effectively within a team environment while maintaining attention to detail under pressure. Excellent communication skills combined with a compassionate approach to patient-centered care across all age groups. Pay:
$31.00 - $35.00 per hour
