The Licensed Practical Nurse, under nursing supervision and direction of an RN, implements the nursing plan. The Licensed Practical Nurse is supervised by the Community Health Nurse (CHN), Home Health RN, and or the Clinical Manager. The job duties and responsibilities are to be performed while observing the following standards: job standard (knowledge, ability, skills), technical skill, quality, quantity, judgement/problem solving, decision making, and initiative. The responsibilities include, but are not limited to the following: Provides nursing care under the supervision and direction of an RN which includes: Administration of medications and immunizations. Observe, measure, record and report clinical data relating to patient's health status. Perform clinical procedures such as, catheterizations, trach suctioning and dressing changes. Provide emotional support and health teaching to patients and families. Communicate with primary CHN/Home Health RN and other professional disciplines, tolerance, progress compliance and deviations from plan of care. Maintain current, complete and accurate clinical records per Schofield Certified Home Care policy and regulatory guidelines. Deliver patient care, treatments and medication administration to assigned patients. Oversee the HHAs and PCAs. Perform other related duties in accordance with SCHC policies, as required by their manager.Working Environment:
The LPN is subject to outside environmental conditions as well as conditions found in patient's homes. The hours of work are determined by availability, work demand and patient need. srnpQualifications:
Education and Training:
Graduate of a NYS approved LPN program. Current New York State Licensed Practical Nurse Registration. Experience - Two (2) years experience in a Home Health Agency (CHHA, LTHHCP, or Hospice) and/or Long Term Care preferred. One (1) year previous medical/surgical experience within the last five years. Character - The individual must possess good moral character, patience, and compassion for the elderly, chronically ill and disabled. EEO Statement We are an equal opportunity employer.
