Job Summary:

The Westchester Center for Rehabilitation & Nursing, a leading provider of rehabilitative therapy and long-term care, is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team! As an LPN at our state-of-the-art facility, you will play a vital role in delivering high-quality patient care and supporting our mission to enhance the lives of seniors in our community.

Licensed Practical Nurse LPN Responsibilities:

Provide direct patient care, including administering medications, wound care, and daily living activities Monitor and report patient status to the Registered Nurse (RN) and other healthcare professionals Develop and implement individualized care plans in collaboration with the interdisciplinary team Participate in quality care initiatives and quality improvement projects Maintain accurate and detailed records of patient care and treatment Collaborate with other healthcare professionals to ensure seamless transitions of care Provide emotional support and education to patients and their families
