•6:00 AM What You'll Do Provide compassionate nursing care to elderly and rehabilitation residents. Care for residents with dementia and other complex needs. Administer medications and treatments as ordered. Monitor resident conditions and report changes appropriately. Collaborate with CNAs, RNs, providers, and the interdisciplinary team. Maintain accurate documentation in PointClickCare. Follow resident care plans and infection-control protocols. Help maintain a safe, respectful, and comfortable environment for residents. Requirements Active New York State LPN License NYS CNA Certification as required by the facility Skilled nursing, LTC, rehab, or geriatric experience preferred Comfortable working with elderly and dementia populations Ability to work 8-hour shifts Willingness to float when needed Important Facility Requirements Pre-assignment COVID testing required In-house testing may be required during the assignment Must follow facility infection-control and PPE requirements About the
