LPN Long-Term Care (LTC)Position SummaryWe are seeking a Licensed Practical Nurse (LPN) to provide daily nursing care to residents in a long-term care setting. The LPN will administer medications and treatments, monitor residents, assist with care needs, and support overall patient comfort and safety.
Responsibilities Provide direct nursing care to residents. Administer medications and treatments as ordered. Monitor vital signs and changes in resident condition. Provide wound care and other nursing treatments. Assist residents with daily care needs as required. Document care, medications, and patient information accurately. Report changes in condition to the RN or healthcare provider. Educate residents and families on care and treatment. Follow infection control and patient safety procedures. Collaborate with RNs, physicians, and other healthcare professionals.
Qualifications Active Licensed Practical Nurse (LPN) license. Long-term care, skilled nursing, or geriatric experience preferred. Knowledge of basic nursing care and medication administration. BLS certification required or preferred based on facility requirements. Strong communication and organizational skills. Ability to work effectively with residents and healthcare teams.
