  • 5:30 PM Job Summary We are seeking a compassionate and dependable Licensed Practical Nurse (LPN/LVN) to join an outpatient Family Medicine clinic in Rochester, NY.

The ideal candidate will have at least 1 year of nursing experience , strong clinical skills, and the ability to work independently in a fast-paced ambulatory care environment. This position provides an excellent opportunity for LPNs/LVNs to deliver patient-centered care to diverse populations while working closely with physicians and other healthcare professionals. Key Responsibilities Provide direct nursing care to patients in an outpatient Family Medicine setting. Obtain vital signs, patient histories, allergies, and health information. Assist with patient rooming and clinical assessments. Administer medications, injections, and vaccines according to established protocols. Perform IM, SQ, and oral medication administration. Obtain 12-lead EKG/ECGs and assist with emergency response when needed. Collect and label throat, urine, and other clinical specimens. Perform nasopharyngeal swabs and other routine clinical procedures. Assist with ear flushes and other basic outpatient procedures. Perform medication reconciliation and assist with prescription refill requests. Make follow-up phone calls and communicate with pharmacies and patients. Document patient care accurately in the Epic Electronic Health Record (EHR) system. Maintain patient confidentiality and comply with HIPAA and infection-control standards. Work collaboratively with physicians, nurses, and other members of the healthcare team. Float to other Family Medicine, Internal Medicine, and Pediatric locations as required. Qualifications Current New York LPN/LVN license

  • must be in hand at submission .

Minimum 1 year of LPN/LVN experience . BLS certification required . Epic experience is required . Outpatient, ambulatory care, or clinic experience preferred. Pediatric experience is preferred. Strong communication and interpersonal skills. Ability to work independently in a fast-paced environment. Flexible, dependable, punctual, and adaptable. Strong ability to work effectively as part of a multidisciplinary team. Additional Information First-time travelers are welcome to apply. Travel experience is not required. Local candidates may be considered at a different rate. Floating to other FM/IM/Pediatric locations is required.
