Valid New York State LPN license required Must be authorized to work in the U.S. Reliable, professional, and patient-centered Must maintain professionalism in all written and verbal communications Must have updated medicals/vaccinations or be willing to obtain them Prior home care experience strongly preferred High-acuity home care experience preferred Drivers preferred Completed drug screen or completed Habituation Form Must successfully complete the Alvita Care onboarding process before accepting casesBENEFITS
Competitive pay: $40-$45/hour , based on experience and assignment Weekly pay via direct deposit or check TapCheck early wage access Medical benefits available- Up to 56 hours of paid sick time per year based on hours worked 401(k) with company match
