Job Description
Job Overview Join our healthcare team as a Home Health Nurse (RN/LPN), where your expertise will directly impact patient well-being of the patient. Patient is a pediatric 1 year old male with a primary diagnosis of Cerebral palsy. Patient is NPO with G and J tubes and on continuous tube feed. Patient lives at home with his family. This is an overnight shift typically 8:30 PM to 5:30 AM Monday to Friday subject to some flexibility. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to develop personalized care plans. Administer medications and tube feedings. Utilize electronic health record (EHR) systems( Alora) to document patient interactions accurately and efficiently. Ensure compliance with HIPAA regulations and infection control protocols to safeguard patient privacy and safety. ADLS including getting patient ready for bed and all patient related activities/chores. Experience Background in home health care, long term care or inpatient hospital.
