Full Job Description Salary:
$35 / hourly
Job Description:
Must be Vent, Trach, G/J Tube comped, and have a desire to work with all skilled patients with acuity levels most common to our markets. Must commit to and work a minimum 72 hours per quarter Training hours do not count as hours worked. Only care hours would be considered for the 72 hours per quarter. Understanding of policy in which falling short of the 72 hour quarterly minimum in consecutive quarters, will trigger a transition to non-float status Must be willing to and complete work assignments in multiple home environments Of the 72 hours per quarter, hours should be completed in at least 2 separate homes Understanding of monthly maximum allowance of 100 hours in the same patient home Must be willing to and complete work assignments in multiple home environments that include patients that they have not worked with in the recent quarter/month
Responsibilities:
Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable
Requirements:
Current Licensed Practical Nurse (LPN) for the state in which the nurse practices. Current Health Certificate (per facility Licensed Practical Nurse (LPN) contract or state regulation). Current PPD or Chest X-Ray. Current BLS card. One year prior Licensed Practical Nurse (LPN) experience.
