Competitive Per-Visit Compensation:

$28 per completed in-home visit (minimum 35 minutes)

Responsibilities include:

Deliver care according to physician instructions and the established plan of care Administer medications, wound care, and other approved treatments Monitor patient conditions and report significant changes to the RN Case Manager or physician Educate patients and caregivers on condition management and self-care techniques Document visits accurately using our EMR system Assist with specialized procedures as directed by supervising RNs Ensure regulatory compliance and follow all safety protocols Qualifications Active LPN license in Ohio (or eligible to obtain) Graduate of an accredited Practical Nursing Program Minimum 1 year of LPN experience (home health experience preferred, not required) Knowledge of Medicare guidelines and CMS standards is a plus Comfortable with electronic charting and managing your own schedule Current CPR certification Valid driver's license, reliable transportation, and auto insurance
