Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following physician orders while adhering to HIPAA and clinical confidentiality policies. Assist with complex procedures such as IV infusion, catheterization, tube feeding, wound suturing, and sterile processing techniques. Monitor patients' conditions closely, including acute pain management, airway management, ventilator support, and infection control measures. Collaborate with multidisciplinary teams on discharge planning, case management, and diagnostic evaluation to optimize patient outcomes. Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral health support. Document all patient interactions thoroughly in EMR/EHR systems like Athenahealth or eClinicalWorks to ensure accurate medical records and compliance with long-term care regulations. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Center), nursing homes, skilled nursing facilities, or home & community care settings. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), CPT coding, and pharmacology principles. Hands-on experience with EMR/EHR systems such as Epic, Cerner, or Athenahealth is highly preferred. Demonstrated ability to perform advanced nursing skills including IV insertion/infusion, specimen collection/processing, phlebotomy, and first aid procedures. Familiarity with geriatric care practices and working with individuals with disabilities or developmental challenges is advantageous. Excellent communication skills for caregiver communication and client education; ability to work effectively within a team environment.
