We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our home health care team. The LPN will provide skilled nursing care to clients in their homes under the supervision of an RN. The ideal candidate has strong clinical skills, excellent communication abilities, and a genuine desire to help patients maintain optimal health and independence in a home setting.

Key Responsibilities:

Deliver safe, effective, and individualized nursing care based on a comprehensive assessment and care plan. Administer medications and treatments as prescribed. Provide and manage feeding tube care and tube feedings according to physician orders and patient needs. Monitor patient health status, report changes to the supervising RN or physician, and document care provided. Assist patients with activities of daily living, including hygiene, mobility, and nutritional needs. Educate patients and families on disease processes, medications, and home care techniques. Collaborate with interdisciplinary team members to ensure coordinated care. Maintain accurate and timely documentation in compliance with agency policies and state/federal regulations. Follow infection control procedures and ensure a safe environment in the patient's home. Work 8-hour shifts as scheduled, including every other weekend as part of regular shift rotation.

Qualifications:

Current and unrestricted LPN license in the state of [Ohio]. CPR certification (required). Minimum of [1-2] years of nursing experience, preferably in home health or community care. Valid driver's license, reliable transportation, and proof of auto insurance. Strong clinical, organizational, and communication skills. Proficiency with electronic medical records (EMR) preferred.

Physical Requirements:
