Duties Deliver direct patient care by performing physical examinations, vital sign assessments, and health evaluations with precision and compassion. Administer medications, injections, IV infusions, and other treatments following strict protocols and safety guidelines. Assist with diagnostic procedures such as specimen collection, catheterization, wound suturing, and sterile processing techniques. Monitor patients' conditions closely, including managing acute pain, airway management, ventilator support, and infection control measures. Maintain accurate medical documentation within Electronic Health Records (EHR) systems like Epic or Cerner while ensuring compliance with HIPAA and long-term care regulations. Collaborate effectively with physicians, case managers, caregivers, and interdisciplinary teams to develop personalized care plans. Educate patients and their families on health management strategies including dementia care, Alzheimer's support, nutrition, medication adherence, and discharge planning. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) or equivalent certification. Proven experience working in diverse healthcare environments such as hospitals (including ICU or trauma centers), nursing homes, hospice care, or home & community care settings. Strong knowledge of medical terminology, anatomy & physiology, and clinical confidentiality policies. Hands-on experience with EMR/EHR systems like Epic, Cerner or Athenahealth for documentation and patient management. Demonstrated skills in patient assessment, triage procedures, medication administration including injections and IV insertion/infusion techniques. Ability to work with geriatric patients or individuals with disabilities; experience in dementia or memory care is highly valued. Excellent communication skills for caregiver interaction and client education; familiarity with behavioral health and infectious disease protocols is a plus.
