JOB DESCRIPTION

Sochi Home Health is a Medicare and Medicaid certified, ACHC Accredited agency. We are currently looking for a nurse with clinical authorization experience to join our team. The office nurse will serve as a liaison between patients, their families, and healthcare providers. The candidate will be dependable, reliable, and have a strong nursing clinical background. The candidate will have ability to communicate effectively with clients/patients and providers.

RESPONSIBILITIES

Review all clients referral information and respond to requests and inquiries as needed. Development, implement, and evaluate individualized care plan Experience with OASIS and

NON-OASIS

assessments is also required Experience with prior authorization request and referrals Promote safe delivery of care through effective communication between patients and staffs. Work collaboratively with team members to help meet positive client care outcomes Monitor patient's condition and communicate all pertinent clinical information to home care team members as indicated Assures proper maintenance of clinical records in compliance with local, state, and federal law Provides patient/family teaching regarding disease process, prevention, infection control, home safety, medications and side effects Direct and supervise the delivery of home care services by LPNs, Home Health Aides and ensures care plan compliance Maintain appropriate documentation with protocols and guidelines of the care management team Notify all disciplines, physicians of discharge and completes discharge summary per agency policy Responding and reporting to changes in patients condition Attending all required in-service trainings and educational events Perform any other job as assigned

QUALIFICATION

Graduate of an accredited school of nursing Current Ohio Nursing license in good standing At least 2 years clinical experience in health care setting Must be self-directed and able to work with little supervision

Skills:

Must have strong clinical nursing background Must have current BLS certification Must pass drug and background test Work hours are Mondays through Fridays 8: 30am - 5pm, no weekend or holiday Each office nurse is on call one weekend in a month. Call our office at (937)434-4004 with any questions.
