Job Description

Licensed Practical Nurse
PT LICENSED PRACTICAL NURSE PT QUALIFICATIONS
Minimum:
Must maintain minimum LPN licensure requirements according to Ohio State Board of Nursing. Must have knowledge of nursing of individuals with developmental disabilities. Must be certified to dispense medications (NAPNES). Must have knowledge of nursing process and assessment skills. Must be certified in CPR. Must be able to use email and access agency website and internet. Must have a current valid Ohio driver license and an insured personal vehicle for agency use. Must possess and maintain a good driving record. Must have a record of reliable work attendance. Must have demonstrated continuous satisfactory job performance. Must be able to meet all requirements and obtain all certifications required to perform the job or any aspect of the job, and to access funding for performance of any aspect of the job.
Preferred:
Minimum of three (3) years of nursing experience, part of which is with persons with developmental disabilities. Previous experience in a Medicaid/licensed facility. Working knowledge of Microsoft Word.
RESPONSIBILITIES
Provide emergency care for illnesses and injuries. Administration of medications. Assure accuracy of all medical records on all individuals during time on duty in accordance with Medicaid and Ohio DODD regulations. Work closely with Registered Nurse Supervisor as part of medical team. Evaluate residents' physical/medical problems in coordination with supervising Registered Nurse and screens for communicable diseases. Develop and provide inservice training to staff, individuals and others as relates to plan of care and IP requirements. Assist with direct care of individuals. Bathing of non-ambulatory individuals. Aid in positioning of non-ambulatory individuals, toileting, and assisted feeding. Be part of team-teaching team. Administer prescription medications and treatments to individuals. Participate in the delivery of services through Individual Habilitation Plans (IHPs). Transport and assist with medical appointments. Coordinate and schedule medical appointments as required. Maintain medical records and follow-through with recommendations. Be on-call via a beeper/phone system for delegated services, as assigned. Responsible for nursing stock items and medication availability.
HOURS:
16 hours/week. Flexible as needed.
SALARY:
Level 5 Nurses
PROBATION
280 calendar days (initial); 90 calendar days (non-initial)
