Job Description

Come Join our Friendly Team Consumer Support Services is currently hiring for the position of LPN in our ICF/IID setting in Heath, Ohio. This position is scheduled Monday through Friday, while also being on-call to address any medical concerns that may arise.
The essential job duties are:
Responsible for daily oversight of a maximum of 20 individuals in the ICF/IIF setting. Oversee and/or coordinate medical care and treatment to the individuals as required, per doctor's orders and the IHP. Provide professional nursing services to each individual; performing basic therapeutic and preventative nursing measures according to established protocols. Maintain medical records and ensure compliance with all state, local and federal guidelines. Serve as a liaison with individuals, physicians, pharmacists, agencies and significant others; including attending medical appointments with the individual. Meet with applicable representatives from any regulatory agency regarding all medical aspects of the company. Provide in-service training to staff in compliance with regulatory agency standards as assigned. Provide oversight for medication administration as assigned and ensure that all medications are administered, dispensed, stored and disposed of by the staff in compliance with regulatory standards and company policy. Participate in and oversee a preventative program for infection control. Monitor the individual for adverse reactions to medications and report to the physician. Purchase and/or requisition needed medical supplies and equipment to ensure an adequate supply is available. Participate in and provide input at team meetings to ensure optimum development and health for each individual. Generate all IHP forms for all individuals while attending the IHP meeting. Complete all assessments as required. Participates in quality assurance reviews in all areas of individual care. Communicate consumer health care situations to the County Director, Program Director, Program Coordinator, Supervisor, QIDD, and County Board as necessary. Act as a resource for clarification and interpretation of health care issues. Adhere to all safety standards set forth per regulatory agencies and company policy.

$52,000.00 per year
