Licensed Practical Nurse (LPN) -
Hospice Care Location:
Dayton, OH and surrounding areas
Schedule:
3x12-Hour Shifts/Week (Includes Every Other Weekend) Join a Hospice Team That Leads With Heart—For Patients, Families, and You At Day City Hospice, we are a locally operated, Ohio-based hospice provider committed to delivering compassionate, patient-centered end-of-life care . That's why we offer manageable caseloads, realistic expectations, and a collaborative culture —so you can focus on what matters most: delivering meaningful care to patients and families. Schedule & Work-Life Balance Three 12-hour shifts per week Includes every other weekend rotation Four days off each week for improved work-life balance Key Responsibilities Provide direct patient care in accordance with the State Nurse Practice Act and hospice standards Conduct comprehensive nursing assessments and implement individualized plans of care Deliver skilled nursing services focused on comfort, dignity, and quality of life Administer and monitor pain and symptom management interventions Educate patients and families on disease progression, care plans, and symptom control Document patient care accurately and timely within electronic medical records (EMR) Collaborate with the interdisciplinary team to ensure coordinated, holistic care Evaluate patient responses and adjust care plans as needed Required Qualifications Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program Current LPN/LVN license in the state of Ohio Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred) Current BLS certification Strong clinical, communication, and organizational skills Valid driver's license , reliable transportation, and proof of insurance Understanding of hospice philosophy and end-of-life care Preferred Qualifications Experience in hospice or palliative care Familiarity with home-based care and EMR systems Benefits & Perks Supportive, team-oriented culture Medical, Dental, and Vision Insurance (effective first of the month) 401(k) with generous company match Generous Paid Time Off (PTO) and paid holidays Mileage reimbursement Tuition reimbursement and career development opportunities Hospice certification support + salary increase upon completion Employee referral bonus program Merit-based pay increases Night shift differential pay Employee discount programs Why Join Day City Hospice?
