Job Overview We are seeking a dedicated AL-LPN (Licensed Practical Nurse) to join our healthcare team. The ideal candidate will possess a strong background in nursing, with experience in various care settings including long-term care, assisted living, and acute care environments. Duties Provide comprehensive nursing care to residents, including medication administration, wound care, and vital sign monitoring. Assist with patient assessments and develop individualized care plans in collaboration with the healthcare team. Administer IV infusions and manage acute pain or complex medical conditions with precision and compassion. Support residents with activities of daily living, ensuring their comfort and safety at all times. Maintain accurate medical documentation within electronic health record (EHR) systems such as Epic or Cerner, adhering to HIPAA confidentiality policies. Collaborate with interdisciplinary teams on discharge planning, case management, and family communication to optimize resident outcomes. Ensure compliance with long-term care regulations, infection control protocols, and safety standards across all duties. Experience Valid LPN license with current state licensure required. Prior experience working in long-term care facilities, skilled nursing facilities, or assisted living communities preferred. Critical care experience such as ICU or emergency medicine is advantageous but not mandatory. Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth is highly desirable. Experience working with individuals with developmental disabilities, dementia, Alzheimer's disease, or memory care needs is a plus. Knowledge of IV infusion techniques, medication administration, and basic life support (BLS) certification required. Demonstrated ability to perform physical examinations, vital signs assessment, specimen collection, and sterile procedures confidently.
