Full Time (36 hrs/wk)GENERAL SUMMARY
Works under the supervision of the Registered Nurse. The LPN's primary job function is to assist the RN in assessing, planning, implementing, evaluating, and supervising individual patient care on a nursing unit according to unit policies and procedures. Assists the RN with implementation of plan of care. Supports each strategic value by providing excellent care to those we serve. Performs other duties as assigned.QUALIFICATIONS
Education:
- High School Diploma or successful completion of an equivalent High School Exam Required
- Graduate of an accredited school of practical nursing required
Licensure:
- Licensed to practice as an LPN in Ohio required
- BLS within 90 days of hire required

JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS
The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time. 1. Assists in completion of the nursing assessment of patients and documents appropriately on patient record. 2. Assists with developing, evaluating and updating a plan of care based upon the assessment of the patient. 3. Assists with the discharge planning process. 4. Provides, delegates, and coordinates patient care. 5. Facilitates patient/family teaching as appropriate. 6. Participates in hourly rounding and bedside shift report. 7. Answers call lights and responds to patient needs. 8. Performs total patient care. 9. Pre-cleans, decontaminates and transports contaminated instruments according to policy. 10. Performs intermediate-level disinfection and/ or high level disinfection per the Instructions for Use (IFU) by the manufacture. This includes ensuring the scopes or probes used within the department are cleaned, disinfected, maintained and stored per the IFU's 11. Performs other duties as assigned. Why SOMC?
