Job Summary We are seeking a compassionate and highly skilled Licensed Practical Nurse (LPN) to join our dedicated healthcare team. The ideal candidate will possess extensive clinical expertise, particularly in acute and long-term care settings, and demonstrate a strong commitment to delivering exceptional patient-centered care. As an LPN, you will play a vital role in providing direct nursing care, supporting patient assessments, and ensuring compliance with healthcare regulations within various community and facility-based environments. Duties Provide comprehensive nursing care to patients across diverse settings such as long-term care facilities, assisted living communities, and home health environments. Administer medications, including injections and IV infusions, following prescribed protocols while monitoring patient responses. Conduct physical examinations, vital signs assessments, and patient evaluations to inform clinical decision-making. Assist with medical procedures such as catheterization, sterile processing, and specimen collection/processing. Maintain accurate medical documentation within electronic health records (EHR/Epic systems), ensuring compliance with HIPAA and confidentiality policies. Educate patients and caregivers on health management strategies, including medication administration, nutrition, and disease-specific care such as Alzheimer's or dementia care. Collaborate with interdisciplinary teams to develop discharge plans, coordinate case management efforts, and facilitate seamless transitions of care. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or region. Proven experience in acute care environments such as ICU or emergency medicine settings is highly desirable. Demonstrated proficiency with EMR/EHR systems like Cerner or Athenahealth for accurate documentation. Strong knowledge of medical terminology, physiology, and nursing best practices related to geriatrics, pediatrics, and behavioral health. Experience working with individuals with developmental disabilities or complex medical needs such as ventilator management or tube feeding is preferred. Ability to perform venipuncture, injections, catheter care, and other clinical procedures competently. Excellent communication skills for effective caregiver communication and patient education. Knowledge of long-term care regulations, infection control protocols, and HIPAA compliance standards.
