You will oversee the daily operations of our designated center, ensuring the highest level of care and support for our participants. In this role, you will be responsible for assessing the care needs of our participants and collaborating with care staff and caregivers to develop and manage personalized care plans. Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions. Perform comprehensive nursing assessments and/or home visit evaluations of participants that considers physical, psychosocial, and environmental conditions. Formulate plan of care designed to promote or restore health. Regularly review care documentation and provides supervision, guidance, and training as needed to achieve quality care. Modify and update the plan of care to reflect progress toward outcomes. Participate in educational programs, conferences, and meetings to stay up to date on current health care practices. Practice nursing with respect for individual, cultural and spiritual differences. Effectively communicate with participant's personal physicians and caregivers regarding their condition. Maintain participants' charts and ready for audit at all times. Keep participants' physician's orders up to date annually. Be familiar with established emergency procedures. Required Education and Experience Prior Supervisory experience in business, community health setting with geriatric patients or equivalent. Strong assessment and documentation skills. Excellent leadership, communication, and organizational skills. Graduate of an accredited school of nursing with one year of clinical experience within the past five years or completion of an accredited refresher course within the past two years Current Oklahoma state LPN license Current CPR and First Aid certifications Strong interpersonal skills with the ability to establish rapport with managers, staff, medical referral entities, participants, and their families. Intermediate knowledge of Microsoft Office Suite, with emphasis on Word and Excel;willingness to learn new programs that will enhance services provided. Additional Eligibility Requirements Lifts, carries, and pushes equipment, or supplies exceeding 25 lbs. Motivate and encourage all participants to engage in activities and socialization. Must be free of limitations which prevent legible handwriting for charting. Must be free of hearing limitations which prevent auscultation of blood pressure, heart, and lung sounds.
